Reimbursement Claim Form                Use the Direct Reimbursement claim form all reimbursement requests

 

 

If your Plan includes Orthodontia coverage, the Orthodontic Treatment Plan Form needs to be completed and submitted in addition to the Direct Reimbursement Claim Form.  This only needs to completed once.
